Social care for disabled children
Herefordshire Council has a dedicated disabled children's social care team, which work closely with disabled children and young people, their families and carers, along with different professionals and agencies, including specialist schools, occupational therapists, child and adolescent mental health services, health visitors, speech and language therapists and many more.
The team offer information, help and support with a range of areas, such as:
- Specialist short breaks and respite
- Supporting the transition to adult services
- Social care needs assessment for disabled children
- Carer's assessment for parents / carers of disabled children
- Direct payments
